Round Bald Patches May Signal Alopecia Areata

Alopecia areata is an autoimmune disease in which the immune system attacks hair follicles. It commonly produces small, smooth, round or oval patches on the scalp or face, although some people develop more extensive loss.

The beard, eyebrows, eyelashes, and other body hair can also be affected. Some patients develop small pits or other changes in their fingernails or toenails. The exposed skin may look normal rather than inflamed.

Alopecia areata is not contagious. Hair may regrow without treatment in some people, but the course is unpredictable, and further episodes can occur. Treatment depends on age, the extent of loss, its duration, and the patient’s medical circumstances.

Dermatologists may use corticosteroids or other therapies for limited disease, while certain Janus kinase inhibitors are approved for severe cases and require professional monitoring.

Scalp Pain, Scaling, or Redness Is Not Typical Pattern Baldness

Classic male pattern hair loss usually develops without significant redness, crusting, pustules, burning, or pain. When those symptoms appear, clinicians consider inflammatory skin disorders, infection, allergic reactions, and forms of scarring alopecia.

Scaling with broken hairs may occur with a fungal scalp infection. Inflamed or pus-filled areas can require prescription treatment. Using cosmetic oils or an over-the-counter hair-growth product without identifying the cause may delay appropriate care or further irritate the scalp.

Psoriasis, seborrheic dermatitis, and contact dermatitis can also affect the scalp. Scratching, aggressive scale removal, and harsh chemical treatments may break hairs even when the follicles remain capable of growth.

Scarring alopecia is particularly important to recognize. In these disorders, inflammation damages and replaces follicles with scar tissue. Hair cannot normally regrow from a follicle that has been destroyed.

Warning signs can include a shiny scalp, loss of visible follicular openings, perifollicular scale, redness, pain, burning, bumps, or expanding areas of permanent loss. Some scarring conditions advance slowly, while others progress more quickly. Early treatment may help stop additional damage, even when hair already lost cannot be restored.

A Dermatologist Looks for the Cause Before Choosing Treatment

A hair-loss evaluation usually begins with medical history, family history, medications, grooming practices, diet, recent illness, and the timing of the change. The clinician examines the scalp, hair shafts, distribution of loss, facial hair, nails, and sometimes other areas of skin.

A dermatologist may use magnification to inspect follicles and hair shafts. Depending on the findings, evaluation can include a gentle pull test, fungal testing, blood work, or a small scalp biopsy. Not every patient needs every test.

The distinction between shedding and follicle damage is important. A condition that pushes more hairs into a resting phase requires a different strategy from an autoimmune disease, infection, broken hair shafts, or permanent scarring.

The American Academy of Dermatology explains the diagnostic process in its hair-loss diagnosis and treatment guide. Bringing photographs, a medication list, supplement containers, laboratory results, and a timeline of health changes can make the appointment more productive.

Hair Loss Treatment Should Match the Diagnosis

Topical minoxidil is available without a prescription and is commonly used for male pattern hair loss. It may slow loss and stimulate some growth, but full restoration is unlikely, and continued use is generally necessary to maintain benefits.

The product label matters. FDA labeling for men’s 5% topical minoxidil warns against self-treatment when hair loss is sudden, patchy, unexplained, or accompanied by a red, infected, irritated, inflamed, or painful scalp. It also advises people with heart disease to consult a doctor before use.

The official FDA labeling for 5% topical minoxidil contains application instructions, limitations, and symptoms that require stopping treatment and obtaining medical help. Using more than directed does not produce faster results and can increase adverse effects.

Finasteride is a prescription tablet approved by the FDA for male pattern hair loss. It can slow loss and promote some growth in appropriate patients, but benefits and potential adverse effects should be reviewed with a clinician. Reported adverse reactions include decreased libido, erectile dysfunction, and ejaculation disorders.

Finasteride can also affect prostate-specific antigen test results, so patients should tell relevant health professionals that they take it. Treatment decisions should consider medical history, other medications, fertility concerns, and individual priorities.

Compounded topical finasteride should not be assumed to have the same approval, labeling, or evidence as an FDA-approved product. In 2025, the FDA issued an alert about potential risks associated with compounded topical finasteride, including reports of adverse events and the possibility of medication transfer to others.

Hair transplants can redistribute existing follicles but do not stop an untreated hair-loss disorder. A reputable specialist should first confirm that the condition is appropriate for surgery and sufficiently stable. Transplanting into an area affected by active scarring inflammation may produce poor results.

Be Skeptical of Products Promising Fast Regrowth

Hair loss is commercially attractive because it can affect confidence and create urgency. Products may use dramatic before-and-after photographs, testimonials, proprietary blends, or claims that one ingredient treats every form of alopecia.

No shampoo, oil, vitamin, laser device, injection, or subscription plan should be expected to treat all causes. A product that helps hereditary thinning will not necessarily treat fungal infection, alopecia areata, medication-related shedding, or scarring disease.

Ask whether a treatment is FDA-approved for the specific use, cleared as a device, prescribed off-label, or compounded. These categories have different meanings. FDA clearance or approval of one product also does not validate every product using a similar description.

Compare the continuing cost as well as the introductory price. Many effective hair-loss treatments require long-term use, while stopping them may allow hereditary loss to resume. Subscription charges, laboratory fees, consultations, and combination products can make the full annual cost substantially higher than the advertised monthly rate.

When Men Should Schedule an Appointment

Arrange a routine dermatology visit for gradual thinning when the cause is uncertain, the loss is distressing, or treatment is being considered. Seek a more prompt appointment for sudden loss, rapidly expanding patches, widespread shedding, beard or eyebrow involvement, broken hairs, scalp pain, redness, scaling, crusting, or pustules.

Hair loss by itself is rarely a medical emergency. Urgent care may be appropriate if a treatment causes facial swelling, difficulty breathing, fainting, chest pain, a rapid heartbeat, or another serious reaction.

The most useful first step is not selecting a popular product. It is determining whether the follicles are gradually miniaturizing, temporarily shedding, being attacked by inflammation, or already developing scars. That distinction determines whether time, medication, infection treatment, or another medical intervention is most likely to help.
